Description (Service) Offer rehabilitation, physical and mental health services in clinic, clients homes, community settings, and remotely through virtual care / telehealth
  • Interdisciplinary team
  • Provide the following services:
    • Health support services - nursing. Private care at home, hospital or other settings
    • Personal support services - homemaking, shopping, meal preparation and accompaniment to appointments
    • Specialized services - Alzheimer, palliative, brain injury and diabetic care
    • Health and Wellness programs - Falls Prevention, Diabetes, Healthy Heart, Osteoporosis, Women's Health, Men's Health, Summer Living (Sun Safety) and West Nile Virus
    • Respite care
    • Friendly companionship

Hours Service: Mon-Sun 24 hours

Eligibility Children; Youth; Adults
